#SRT2026 REALM: Case Submission Form

We are excited to introduce the #SRT2026 REALM Competition, an interactive session dedicated to learning from our real radiology practice.


REALM (Radiology Events and Learning Meetings) focuses on shared learning through discrepancies, diagnostic challenges, perceptual misses, and exceptional diagnostic observations. These cases highlight important teaching points that help improve radiology practice and patient care.


We invite radiology residents attending the conference to submit an interesting case that demonstrates a meaningful learning opportunity encountered during their clinical practice and on-call reporting.


Shortlisted cases will be presented anonymously by our moderators during the REALM session at the SRT Conference in Southampton, and compete for the 'Best REALM Case Award'.

Slides format


  1. Slide 1 – Case Introduction

    • Case title

    • Patient age and gender

    • Relevant clinical history and indication

    • Imaging modality performed


  1. Slide 2 – Imaging Findings

    • Large imaging panel

    • Use 4 images maximum

    • Annotate subtle findings with animated arrows or circles


  1. Slide 3 – The REALM Element

    • The diagnostic challenge

    • Key questions:

      • Missed finding on initial report

      • Subtle abnormality detected later

      • Perceptual or cognitive error

      • Unexpected but important diagnosis

      • Excellent spot that changed management


  1. Slide 4 – Final Diagnosis

    • Final diagnosis

    • How it was confirmed

    • Impact on patient management

    • Optional:

Submission deadline 19 April 2026 at 11:59 pm GMT.

Grade

Please upload a PowerPoint presentation (maximum 5 slides) containing anonymised imaging and key teaching points.


Presentations should contain minimal text and emphasise imaging, consistent with the educational nature of REALM sessions.


Ensure all patient identifiers are removed.
